CLINIC POLICIES
– No-Show Policy:
When a patient initially fails to show without notice for all reasons except miscommunication, the patient shall be phoned and offered a chance to reschedule. The patient will be politely asked to call us in the future, at least the day before the appointment that they are unable to keep, so that others can keep that time slot.
On the 2nd failure to show without notice, a card shall be sent to the patient at the doctor’s discretion.
On the 3rd failure to show without notice, a second different card shall be sent to the patient at the doctor’s discretion requesting the patient to find another provider.
– Insurance Card Policy:
It is now the policy of our office that our patients present their insurance cards at the time of service for every visit. This is necessary for our office to ensure payment from your insurance company.
