To avoid delays, patients may click this link to download and print the Patient Registration packet. All pages must be completed before we can check you in for your appointment. Patients can complete the forms at home and bring them to their appointment or fax them to our office using the instructions on page 1 of the Patient Registration packet. If you prefer, you may choose to come 15-20 minutes before your scheduled appointment and do the paperwork in our office, but please be advised that completing paperwork in office will increase the length of time you can expect to be there.
